GETTING STARTED
In order to make the intake process more efficient, you can download the intake forms and fill them out before your appointment- just click on the links below, print, and fill out.

- Email or call 480 586 6353 to set an appointment time
- Evening and Saturday appointments may be available
- Bring your completed forms to your first session
If you are uncomfortable filling out on the forms, we are able to conduct the intake process during your first appointment. Please do not hesitate to call or email if you have any questions or concerns.
HOW DO I PAY FOR COUNSELING?
Payment is required at the beginning of all counselling sessions. For your convenience, I accept Visa, MasterCard, Debit and cash in payment for all sessions.
Please understand that when you make an appointment, I am reserving that time for you. You will be charged for missed appointments not cancelled 24 hours in advance.
FEES:
1 hour: $125
90 minutes: $150
INSURANCE:
I am a contracted provider with:
MMSI (Mayo Clinic)
Aetna and Aetna E.A.P.
Blue Cross Blue Shield
Please be aware of co-pay, deductibles, and co-insurance fees for your individual policy. These will be collected after insurance has been verified.
I can provide a Superbill to submit for out of network insurance.
